Healthy Families 

   

6726 Arlington Expressway

  • Program Manager: Michelle Stephens 

  • Number of Staff: 11 full-time, 1 part-time 

  • Number of Volunteers: 21

  • Number served: 230 families.
  • Program Goal: To improve the health status of women and infants; build positive parent-child interaction through healthy family functioning; and promote healthy growth, development, safety and school readiness for children under the age of five. 

  • Referral Process: Free and open to the public, there is an Intake process to collect information and a Family Assessment Worker is assigned.
     

   The Bridge Healthy Families program operates throughout zip codes 32211, 32277, 32225, 32233, 32246, and 32250. Program services include intensive Home Visitations enhanced by Parenting, Child Development and Childcare classes.

Healthy Start services:

  • Breastfeeding and childbirth education
  • Infant risk screening
  • Nutritional and psychological counseling

Jacksonville Children's Commission, Healthy Families Florida, Northeast Florida Healthy Start Coalition and the Florida Department of Health support this program.

Healthy Families Outcomes:

  • 95% reported no child abuse or neglect incidences.

  • 100% of families in the program are linked to a medical care provider. 

  • 95% of target children are up-to-date with immunizations.

  • 97% of mothers enrolled for two years have not had a subsequent pregnancy.

  • 96% of the families have established a family goal plan.

  • 90% report development of positive parent-child interaction.
