The Bridge Teen Adolescent Health Clinic Partnership

  • Number Served: 5,852 unduplicated visits
  • Referral Process: Referrals are received from Duval County Health Department, other patients, various local hospitals and agencies and from Bridge  programs
  • Program Goal: Providing comprehensive medical services to children and youth living in at-risk, low income areas.

This long standing partnership between The Duval County Health Department and The Bridge provides at risk youth and families with comprehensive medical services.
